Information request letter updating records

I request copies of the following [or all] health records related to my treatment.[Identify records requested, e.g.medical history form you provided; physician and nurses' notes; test results, consultations with specialists; referrals.][Note: HIPAA also allows you to request a summary of your medical records.Files or records specific to individual employees will be accessed only when needed for legitimate reasons related to an employee's job, consideration for a transfer, promotion or job change, bonus, recognition or commemoration, or as needed to respond to a legal inquiry.

If my request cannot be honored within 30 days, please inform me of this by letter as well as the date I might expect to receive my records*.

University Human Resources is the custodian of employment files and records.

Normally, 24 hours prior notice will be required for employees to view their personnel files.

Electronic records are accessible at any time through the HR/Payroll system.
